WP Link To This Post is a plugin which makes it easier for your visitors to link back to your posts.
It creates an easy copy-paste HTML code after the post content so that a visitor can easily find the HTML code
which can be used in other posts, pages or any part of a website.

- Upload the
wp-link-to-this-postfolder to the to the/wp-content/plugins/directory - Activate the plugin through the ‘Plugins’ menu in WordPress

Yes but you will need to go to the wp-link-to-this-post.php file to be able to do that.
You can change the title by editing the text within<h5>tag. - Can I change the look for the code?

Yes but again you will need to go to the wp-link-to-this-post.php file to be able to do that.
You can change the look by editing the CSS within<p>tag.
